I'd suggest you either make the sleeve heads puffier (like the woman in the steamfashion post), or use the flange, but shorten it, like this Elizabethan jacket (http://www.extremecostuming.com/reproductions/themaidstonejacket.html). I'd also use some kind of closure for the sleeves, probably cuffs.
